The Pilgrims stunned the Etihad to silence in the 38th minute as Maksym Talovierov rose highest from a corner to head home.
The lead did not last for long as 19-year-old O’Reilly dragged Pep Guardiola’s side level moments before the break.
The hosts dominated the possession in the second half as they looked certain to find a winner.
After some stern resistance from the visitors, the Cityzens finally took the lead and it was O’Reilly with his head again in the 75th minute.
Erling Haaland then set up Kevin de Bruyne to tap home in the 90th minute to seal the result.
